Here at Infinity Home Health and Hospice our team is comprised of compassionate healthcare professionals who are committed to understanding the needs of every individual patient. We take the time to understand and discuss each patient's diagnosis, treatment plan, and any other concerns they may have. We prioritize patient safety, comfort, and satisfaction. Our team will develop custom care plans tailored to meet the specific needs of each patient so that they can achieve their healthcare goals.

    Signs it may be time for hospice:​

  • Dysphagia leading to recurrent aspiration

  • Uncontrolled pain, shortness of breath, nausea, or vomiting

  • Decrease in ability to perform activities of daily living such as bathing, eating, and/or walking

  • Progressive weight loss

  • Decline in oral intake

  • Frequent emergency room visits and/or hospitalizations

  • Patient has a terminal illness and a limited life expectancy

   

 End-Stage Chronic Diseases

  • ALS

  • Alzheimer's Disease

  • Cancer

  • Coma

  • COPD

  • CHF

  • Dementia

  • HIV

  • Liver Disease

  • Parkinson's Disease

  • Renal Disease

  • Stroke

Services Provided:

  • Available 24 hours a day, 7 days a week

  • Hospice Nursing Care

  • Hospice Physician / Medical Director

  • Hospice Aides for Personal Care

  • Medical Social Work and Spiritual Advisors

  • Bereavement Program offered for 13 months after the death

  • Pain and Symptom Management

  • Pharmacy Consultation / Services

  • Medical Equipment and Supplies

  • Respite or Inpatient Care (If needed)

  • Continuous Care Requiring Intense Intervention (Eligibility required)
