- Dysphagia leading to recurrent aspiration
- Progressive weight loss
- Uncontrolled pain, shortness of breath, nausea, or vomiting
- Frequent emergency room visits and/or hospitalizations
- Patient has a terminal illness and a limited life expectancy
- Decrease in ability to perform activities of daily living such as bathing, eating, and/or walking
- Decline in oral intake
- Available 24 hours a day, 7 days a week
- Hospice Nursing Care
- Hospice Physician / Medical Director
- Hospice Aides for Personal Care

- Bereavement Program offered for 13 months after the death
